Please bring:
- Parent FSA ID (www.fsaid.ed.gov)
- Student FSA ID (www.fsaid.ed.gov)
- Parent’s 2016 federal and state tax return
- For non-tax filers, documentation of 2016 income
- Student’s 2016 federal and state tax return
- For non-tax filers, documentation of 2016 income
- Student SSN and date of birth
- Parent SSN and date of birth
- Dates of birth for any other members of the household
- Marriage, Divorce or separation date of parents
